P.R.I.C.E. Treatment

Hundreds of athletes sustain acute injuries everyday which can be treated safely at home using the P.R.I.C.E treatment. An acute sports injury is one in which there is a specific mechanism of injury (hit by ball, plant and twist, collision with opponent) and an immediate onset of symptoms including swelling, pain, bleeding, and possible deformity.

A number of types of sports injuries can be considered acute including:
• Sprains (injury to ligaments)
• Strains (injury to muscles)
• Contusions (bruise)
• Subluxation (partial dislocation that reduces itself)
• Dislocation
• Fractures

If there are signs or symptoms of a serious injury (deformity, severe swelling and/or pain), emergency first aid should be provided while keeping the athlete calm and still until emergency service personnel arrive on the scene.

However, most acute sports injuries can be treated safely at home using the P.R.I.C.E. principle. The acronym stands for:

• Protection• Rest
• Ice
• Compression
• Elevation

The principles of P.R.I.C.E. should be used for the first 48 – 72 hours immediately after the injury. The goal during this time frame is to control the amount of swelling to the injured area, prevent further injury, and reduce pain. Following these principles can effectively reduce the amount of swelling in an injured area thereby reducing the amount of time required for rehabilitation.

Protection

The first principle is protection. The purpose of protection is to avoid further injury to the area by protecting the injured structures. The type of protection used varies depending on the injured area but may include an ace bandage, aluminum splint, sling, protective tape, or over-the-counter brace.

These devices are applied so that the injured structures are protected from further injury. For example, an individual who recently suffered an ankle sprain may initially wear an ankle brace. A good ankle brace should protect the athlete from movements of inversion (movement of the ankle/foot inwards) and eversion (movement of the ankle/foot outwards) because those movements could further injure the damaged ligaments, but would allow the ankle/foot to move up and down.

Rest

Rest is the second component of the P.R.I.C.E. principle. The purpose of resting is to allow the body’s own healing processes to naturally occur without being impeded by movement of the injured area. Any increase in movement of an injured tissue results in increased circulation to the area which in turn may result in further damage to the injured tissue and/or increased swelling.

For injuries to the lower extremities, crutches may be a good option for resting the injured area. If an athlete is in enough pain to have a noticeable limp, the athlete should be placed on crutches. The crutches provide a safe means for the athlete to move around while ensuring that the damaged area is not stressed.

Athletes can either be non-weight bearing on crutches (injured leg is bent with full weight of athlete on crutches and uninjured leg while walking) or partial-weight bearing (partial weight is placed on injured leg). Athletes may use a combination of these two gaits starting with non-weight bearing for the first 48hours and then moving to a partial-weight bearing gait.

Ice

Ice is another component of the P.R.I.C.E. principle. There are a number of types of cryotherapy that can be used effectively to treat injuries.

Ideally, ice packs are made of crushed ice because the crushed ice is more comfortable for the athletes and conforms to the contours of the injured area better than cubed ice. Ice can be placed into plastic or Ziploc bags. A light barrier should be placed between the skin and the ice bag (paper towel) to prevent injury to the skin during the application of the ice.

The ice pack can be secured with an ace bandage if needed. The ice should be applied for 20 minutes at a time and then removed. This can be repeated every two hours while the athlete is awake.

Some athletes are hypersensitive to ice or may actually be allergic to ice. In this case, the athlete’s skin may become red, raised, and blotchy where the ice made contact with the skin. If this occurs, immediately remove the ice pack and let the area rewarm.

Compression

When the ice pack is removed, a compression wrap should be applied to the injured area. The compression wrap serves as a mechanical barrier so that swelling is minimized in the injured area. There are a number of compression wraps available on the market, but the most commonly used is an elastic or ace bandage.

Elastic bandages come in a number of sizes (2”, 3”, 4”, and 6”) and should be selected depending upon the body part that needs to be wrapped. A two inch elastic bandage is ideal for an adult’s wrist/hand or a child’s ankle.

Three and four inch elastic bandages can be used for an adult’s ankle or elbow. Six inch elastic bandages are perfect for the application of compression to the thigh for a quadriceps or hamstring injury.

To wrap the knee, groin, or shoulder area, the preferred elastic bandage is a double length elastic bandage. This double-length wrap can be purchased in four and six inch widths.

Elevation

The last component of the P.R.I.C.E. principle is elevation. Elevation is important immediately post-injury to reduce the amount of blood flow to the injured area. For the lower extremities, the athlete can elevate his/her leg by lying down and elevating the injured limb on pillows. The key is that the athlete needs to have the injured area above his/her heart level.

Following the PRICE principles is an effective way to minimize the swelling in an injured area so that the athlete can return to play quickly.

The five-step process for treating a muscle or joint injury such as an ankle sprain is called "P.R.I.C.E." which is short for Protection, Rest, Ice, Compression, and Elevation).

It can be used even by someone without first-aid training, and should be used immediately when an injury occurs – the earlier, the better – while further medical attention is being sought.  

If the athlete experiences too much pain during the process, stop immediately.

"P"  is for Protection

Protect the injured person and the area being treated but also protect yourself.  If the injury occurs on the sports field, stop the game.  

Protect the area being treated with a splint if possible.

If the athlete can move, carefully move them to a safer area using a stretcher or a crutch, but if there is any doubt, do not move the injured athlete.

"R" is for Rest

When a child is injured, the body responds in an effort to defend, localize, protect, and clean up the injured area, a response called inflammation.  At the time of injury small blood vessels at the injury site rupture and cause tissue bleeding, which, in turn, can cause bruising and swelling.  

There are five warning signs of inflammation:

  • Pain
  • Redness;
  • Tissue hotness;
  • Swelling, and
  • Loss of function.

Not every injury exhibits all these signs, but if your child is in pain then it is important to stop exercising immediately to avoid further damage.

"I" is for Ice 

  • As soon as possible after injury, begin applying ice continuously for the first 15-20 minutes to decrease swelling and pain.
  • An ice bag can be applied directly to the skin (except on the outside of the knee) unless the child has a known cold allergy. 
  • The safest form of icing is to use a plastic bag with ice cubes or crushed ice, or instant cold packs.  Do not use “blue ice” or “gel packs”.
  • While icing will be uncomfortable at first, in 2 to 3 minutes, the skin will go numb and the ice bag will feel more comfortable.  
  • If the child is very uncomfortable with ice bag, then a barrier like a wet towel can be placed between the ice and skin.
  • The ice bag can be secured in place using an elastic bandage, but the compression should not be too tight.
  • During the application of the ice, ask the child to wiggle their fingers/toes and monitor tissue around area that is being iced to ensure that they are not experiencing a lost of sensation.

Icing for 15-20 minutes with an ice bag is unlikely to cause nerve damage or frostbite; however, any loss of sensation and any changes in tissue coloration in areas other than the area that is being iced indicate that the application is no longer safe. 

Because ice and plastic bags are so important, make sure that someone brings these to every practice and game. Better yet, to be on the safe side, bring your own!

"C" is for Compression

  • After icing, to further prevent swelling, wrap the injured area with an elastic bandage or compression sleeve.  
  • Take the wrap and start farthest away from the injured body part to the heart.  
  • Continuously wrap, always making sure to cover half of the previously wrapped area.  
  • As you get closer to the end of the wrap, you don’t need to wrap as tightly.  
  • You will know you have wrapped correctly when the hurt part ends up in the middle of the wrap.  
  • Tension should be firm but still comfortable. Tingling or pain means it’s too tight.  
  • Incorrect application of an elastic wrap could cause further pain and damage and delay healing. 

"E" is for Elevation

For both upper and lower limb injuries, it is equally important to keep the limb elevated to minimize swelling.  Elevation to the level of the heart or above allows for excess fluid to be pumped back into the blood vessel system and will help prevent further swelling from occurring. 

Return to play

In addition to protection, rest, ice, compression, and elevation, restoring range of motion (ROM) is an important part of the recovery process when the swelling and pain begins to subside.  Active ROM of an injured joint assists in introducing new blood flow to the injury site to "flush" out any residual cellular waste and joint edema present.   Motion also assists in decreasing scar tissue and adhesion formation and stimulates the nervous system to begin activating the muscles that control strength, balance, and proprioception (position-movement sensation or body awareness) when return to play occurs.  An athlete experiencing pain or swelling upon return to play or during rehab prior to play should seek the advice of a physician for further evaluation and/or diagnostic testing.

Return to play should ultimately be dictated by:

  • pain-free, full range of motion;
  • equal strength compared to the uninjured side;
  • elimination of bruising and swelling;
  • equal balance compared to the uninjured side (if it is a lower body injury); and
  • no residual gait deficits. 

Not only is rest important in the period immediately after your child is injured, but adequate rest is necessary before your child returns to sports. Returning too soon from an injury can cause further damage and result in a chronic, long-term condition, such as chronic ankle instability

Medical native watercraft marvel 12 kayak and costs of sports-related injuries in a total population

A prospective study of acute injuries from sports and physical exercise was carried out during 1 year in a total population of a municipality with 31,620 inhabitants. The medical treatment and the services required for the injuries have been calculated and related to the total consumption of medical care in the municipality, sports med prices. A total of 571 injuries (17% of sports med prices injuries) occurred in 28 different sports: 65% of the injured were males. Sports injuries accounted for 3% of all acute visits and there were altogether 1,083 outpatient visits, which yields a mean of 1.9 visits per injury. Related to the total consumption of outpatient visits to the five clinics in the municipality sports injuries also accounted for 3%. Forty-four patients were hospitalized; the proportion of inpatient care due to sports injuries was 0.7% and the mean length of stay in hospital 3.9 days. The total amount of sick leave compensated for sports injuries (3,477 days) was 1.2% of all days compensated in 1984. The overall mean cost per injury was US$ 335. Individual sports (motorcycling, downhill skiing and equine sport) were by far the wheaton college ma baseball division costly in the Falköping survey, the dearest of the team sports ranking only fifth (handball followed by soccer).

Sportsmed: End of an era

 

After over 30 years of service to the Canterbury sporting community, the directors and shareholders of Sportsmed have unanimously decided a change of business direction and model is required. Sportsmed will cease to trade on 8th October 2021 (Covid restrictions permitting) and the individual business and clinical groups currently within Sportsmed will respectively embark on new enterprises.

The Sportsmed physio’s and doctors will relocate to 136 Moorhouse Ave and trade as Motus Sportshub ( www.sportshub.co.nz) and the Sports doctors ( www.sportsdoctors.co.nz) respectively.

The Sportsmed podiatrists and Sportsmed chiropractors will remain at 156 Bealey Ave and both rebrand as The Sports Clinic. (www.sportsclinic.co.nz).

The directors of Sportsmed wish to thank all our valued patients and referrers over the years, and whilst sad to see this iconic brand finish, they are equally excited about what the future holds.
